Die preparation is an age old system. The dies are created the same way and used in the same way. Die carcks are a very common occurence, and they often crack in the same places, as the dies are created and prepared in the same fashion.
If you read the other post made by errorist that includes the article by Ken Potter, the article answers these questions. Proof coins are struck at much higher pressures and the dies are prepared differently. They tend to have the same issues over and over again.
