PCGS Submission form, I having trouble filling out the last part number 9 fee calculation .

I'm new to this, can someone please take a picture of their PCGS Submission form and email it to me ,I just need to see a sample . Cross out ur personal information . Ty so much for your time and much appreciated

If you go on the PCGS website and click on contact I believe they will send you a package with submission forms in it. They sent it to me about 4-5 years ago. Don't know if they continue doing that but it doesn't hurt to try.

 

Google PCGS Submission form. Open the first one that says PCGS online submission form. scroll down on the right side to Step by step submission guide. It has the submission form with instructions.

The online submission forum on the PCGS website is much easier.

The paper version just go line by line with the charges. Do you need oversized holders if no put 0 0. Do you need to renew your membership if no put 0. Extra services like variety or TrueView if no put 0 ect.

Then if you insist on doing it by hand put a note at the bottom saying 8 vouchers
