Die Deterioration.. When a US Mint employee checks the die and the coins that it has struck (several thousand) and sees the worn die strike. They stop the minting equipment and change the die for a new one. They could go through several dies throughout the year.

Not really overachieving but overuse

 

Definitely a worn die

 

Lazy then working on commission

 

Guessing this one probably shouldn’t have made it past quality control

 

They can’t inspect them all.
