I'm not 100% sure exactly what it is, but I believe it's some kind of token that commemorates 12 years of service in the German army. I've seen similar ones in silver with a 4 instead of a 12. It's kind of a miniature version of the "long service" medals issued during WW2. Translation of the legend is: treue Dienste In Der Wehrmacht "faithful service in the armed forces" I think you confused the S for an L which is easy to do in that script.
