I recently saw an error coin that was very unusual. I'm sorry I don't have a pic. It was a lincoln 1c and it was off-center. The off center part was bent back and the copper was scraped off, so you could see the zinc underneath in some spots. Could it have been caught in the riddler? It also looked like the die pressed and then dragged, scraping it.
    What happened?

Sounds to me like you described what happened pretty well ;)

Could be post mint damage. A lot of off center coins will have post mint damage due to getting caught in coin rolling machines. This causes the machine opperator to get a screw driver and pry the coin out of the machine.
I always used to wonder why so many wheat o/c's had huge gouges in them and one day it finally came to me as to why.
