Is this a mistake after mint or during. There is a little flaking but 2 large indents the one you can see the other is right around the queens neck and a little larger than the bottom one
It looks like a lamination error to me, part of the planchet flaked off before it was struck due to impurities. If it were done outside of the mint I don't think those dots on the border would still be there.
