1986 D penny struck through and retained?

    Can anyone identify these retained struckthrough errors? My research brought me to a screw and 2 bearing washers.

    Post Mint Damage.. Someone struck the Cent with a blunt object.. That's no screws nor washers.

    Looks like a collapsed plating blister or possibly some zinc rot. Doesn't look like a retained strike through

    Punch mark and other a die chip?

    On the bust the mark is shaped like a hexagon. Does plating blisters take that shape? What would cause that?
