I don't know how this could have happened at the mint but I figured I would ask anyway. Could this be an error?
DEFDAM - Definitely Damaged Not a Mint Error of any kind. I'm thinking a hot soldering tip or some kind of electrified wire was placed on it to cause it to appear the way it does.

Looks like it was melted, or attempted to be. It's common to use some type of instrument to poke and stir while melting down alloys to extract the precious metals. God knows I have enough friends that will melt anything they can to make bars and ingots.
Only human error. As in a coin that was fine when it left the mint and after 70 years of abuse it looks like this. The only thing that coin has going for it is the silver in it. Just plain PMD.
