Obverse appears to be Patriotic Die#174, and the reverse Store Card Die #1246. My copy of Standard Catalog of US Tokens is out of date, but it lists the rarity of this token as R6, with a value of roughly $50. Like I said, mine is the 2nd Edition, from 1997, so, prices have undoubtedly changed from that figure.
I believe the newest edition is the 4th, printed in 2004. I haven't seen a newer. It is an excellent reference on Civil War tokens as well as old store cards, etc.
