    It seems as though the back has has a gas pocket.

    The coin was probably heated causing a small gas bubble to expand.

    if it was heated ,it will damage the whole coin it seems to be an extra metal in this spot

    Yep, heated til the metal softened and a trapped gas bubble expanded.
