It looks like it could be 2 heavy cuts. If the cut is at a shallow angle, the displaced metal doesn't move much. Over time, circulation wear pushes the displaced metal down, which can cover some/most/all of the original cut but still leave a raised area. Notice how the coin has other scratches/cuts on both sides Edit to add There was a thread 2-3 years about a coin with a raised X in the field. It was shown to be caused by what I described above. Graffiti, is the only thing that comes to mind when I see that coin. I define graffiti as intended damage.
