Struck while the lamination was attached, then the lamination separated afterward. It's still definitely a legitimate error, not PMD, if I'm right.
As noted above, it's a lamination on the planchet (due to improper metal mixture) that detached after the coin was struck, and in circulation. Not a cracked planchet -
