Did some research and the closest description I can find for this is Push Doubling. Am I correct? Thanks for any opinions!

Push doubling is a subtype of machine doubling. Slide doubling is another. The rarest is rim-restricted design duplication.
These are on the dies, though, right? They're movements tantamount to strike doubling but imparted on these single-squeeze dies, is that right?
from what error-ref says, it's a bounce. The die hits, bounces and then hits again slightly offset. I guess that would imply more of a problem with the press itself instead of the die? Something's loose somewhere... http://www.error-ref.com/push-doubling/
