    How do they do that? I imagine one would somehow affix the coin to some surface which would hold it in place, and then use tools akin to a hammer and chisels of various types to work the metal around, rather than removing any metal.

    Actually it is a bit of an optical illusion - most of the design is deeply incuse and nothing is higher than the original surface, so not as difficult to do as you might expect.
