Any help would be appreciated. There are only 6 of these large size 1902 $5 plainbacks known, from what I understand, from Scottsville, VA.

It is FR-607 but Nationals are not collected by Friedburg number. Every Elliott-Burke $5.00 plain back regardless of issuing bank has the same FR number.

Filling out the PMG form, it asks for the "catalog number" which is what I assumed the FR # was. Am I wrong? (It says they charge $5 if you don't fill out the catalog number).

Also, the signatures, "Dorrier and Dorrier.".... Father/Son?

Yes. The catalog number means the Friedburg number. The additional charge is if they have to look it up.
According to my book on US currency, @letlow is correct. It is FR-607.
