2009 p formative year penny chip out it

I found this big hole in the coin (first arrow) and also an extra thumb. I know there are four different varieties, I believe this is the forth variety. How did this chip of copper fall out?

It might be a struck through or post mint damage. Can't tell from the picture.

I took another image of the chip. Does this help? Also, I know this is a double thumb error, does the chip out take away from any value or add to it?
