Found in mint bag of Oregon quarters. What would cause this and is it considered an error?

Only a guess but it might be a strike through error.

Can you provide a scan of the whole coin? My first instinct is that you're looking at post mint damage possibly caused by someone's keys.

Possibly - but it kind of appears as if the pointed sections were pushed up and over the incuse area - which would mean it occurred at the strike.

Coin was uncirculated. It came in bag directly from mint.Maybe it's from rim hitting coin while bagging?

I would think it's just contact damage that occurred after the strike. These appear to be reeding impressions, perhaps impressed at a shallow angle.
