Looks like a die chip to me no RPM and below the D looks a little like lamination but not sure

I disagree, it looks very similiar to the 1959D-1MM-007 shown on coppercoins.com. I may be wrong about the exact variety, but it is a D over D:mouth:

 

The part above the D appears to be grime. The part below the D looks like dirt or corrosion. The d itself appears damaged, not doubled.
