It takes steady surgical hands Im sure. Think about how tiny the movements have to be to create that even under magnification....very difficult. Ill say this just because Im full of it today....back in the day I used to sell and maintain diamond blades to Ophthalmologists. These doctors would work with incredible skill and precision making what is called scleral incisions (they dont do this procedure much anymore as they enter through the limbus right outside the cornea which is more forgiving). I sat in on too many to count and some of these guys had hands of Gods. You hear about it but cant comprehend the tiny, delicate and perfect cuts these dudes would make. They would run a cataract surgery in about 10 minutes like it was as easy as riding a big wheel. I wish my hands were as smooth. Im telling you, those surgeons would appreciate that coin. Too much emphasis in the current generation on non "tangible" things like computers and soforth. I love seeing good mechanical skill.
