Worn Die Issue.

 

Your third photo appears as have been struck through grease or other debris. It was hard to tell on your first two pics. Minor and not really an error per se, more a maintenance issue at the Mint from not cleaning properly.

 

Common problem with the quarters. I've posted before about them and it was determined to be grease left over on the die and then struck. https://www.cointalk.com/threads/finding-a-few-of-these.327469/

You can find some really neat patterns on the quarters.
