I found this 1995-D nickel with no initials on it. It is very wrinkled looking all over it on both sides. I'm assuming that the wrinkled look come from machines and so forth. Is this a error coin with no initials ?

Members of Coin Talk use different terms for coins like this. Parking lot finds, road rash, def dam, etc. Worth 5 cents. The initials FS are faintly visible below the bust.
