Can anyone tell what image has been punched over the rev, and any opinion on the color and issues on both rev and obv. Thanx a bunch!
You have a well circulated Quarter with damage.. Not a Mint Error Circulation wear and tear & Environmental damage
No image has been punched over the reverse. The coin has taken a lot of hits in circulation and also has environmental damage. That's all.

Hi Welcome to ct! Like @paddyman98 and @Kasia said, post mint damage. It actually looks like a pop can lift lid was hammered into it or a c-clip.
