I'm fairly certain you're looking at PMD (Post Mint Damage). Based on both sides of that coin, this coin appears to have been through hell and back again. Looks like it was exposed to some pretty good heat. Perhaps a soldering iron, a heat gun or something like that. Unfortunately, in my opinion, this is not a mint error, just plain old damage.
The mint puts out some crazy badly minted coins at times, mostly from worn dies, but nothing like that. Sorry, this is totally pmd.
