Well, the obverse has the initial strike that has its original designs now totally struck over by the second strike. All you see from the first strike is what you see on the second photo down.
The reverse only has 1 image that is stretched out from the 2nd strike. Most likely because another planchet was between. Is this a fair observation?
Since it did not eject properly after the first strike, a second planchet was now in there with it underneath the reverse of this one. I am guessing this coin stuck to the obverse die and upon the second strike it shifted position. As it came down the force coming down smacked it against the collar unevenly causing it to bend and cup. What do you think @paddyman98
Thanks for the additional info, JCro57. It all makes sense now. Looking back at your 3rd photo, there seems to be a placeholder for something in the center. Probably is the second planchet.
