If the dies were loose [both]. Then yes, it can happen. They are tools, and if not properly locked into their holders, then they can move under the pressures and movements of the machinery.

YES! If you have just one screw loose on that equipment that you drive, but you then all are loose or missing. Will it affect your job performance? Think of it as a coin tooling doing the job that you are asking about.
I can see this being true in what you are referring in machining but not in a multiple squeeze hubbing process! If I'm wrong please let me know! BTW not saying this is hub doubling I'm just looking for an answer I can agree with!
You posted "is MD possible on both sides of a device?" and I think it was taken as "is MD possible on both sides of a coin". JMHO
I think I answered this already. MD happens when the die is loose, and "rattles" against the coin after the strike but before the coin is ejected. If it is very loose, it can rattle in such a way as to hit the coin multiple times.
Ok if this were the case wouldn't we see some difference in the image on one side or the other of the device as the dies moved (slides) as in the top of the T in my first pic ( to the north). E and W appear the same on the A in my pic. So wouldn't the A being so close to the T resemble the same MD. Sorry just out of hand surgery and looking for a good answer! Ouch !

The hubbing process occurs before the minting process...I think. And I thought it was "hub doubling" that was the true doubling.
MD is possible in any direction at any time, and capable of being in all 4 directions. But only if DDD is involved.
Yes sir it is. But all still happens in the minting process as we've seen people argue! True doubling happens in the hubbing process weather in the single squeeze or multiple squeeze hubbing processed will determine the class of hub doubling. I.E I,II,III,IIII,V in class. If I understand it correctly!
