    I would put my money on an overly aggressive coin roll crimping machine.

    Wouldn't that only damage the side closest to the crimper?

    Yes.... I thought about that. But then it’s had eighteen years in circulation. I could see the obverse and reverse facing a coin roll crimper over the course of its life.
