He uses "repousse". I doubt anything has been added, but he does not show the back of the coin. My guess i9s that it is a hole. If you like this type of design, look at Wendell August Forge - WAF LINK -. They do not use coins, but it is the same type of manufacture.
I noticed the final value. Take an almost worthless cent, emboss an image, and now it is worth several dollars. Not bad for a damaged coin. Reminds me of the hobo nickels.
