What would cause the " 4 " in the 1894 to be so out of position from the " 189 "?????

I cant even see a date in the picture you provided. If you could possibly provide clearer photos you'll recieve much more accurate answers. Thanks

From what I can see in your picture:
I believe that there is a die break that extends from the 4 in the date to the Indian's bust.
Not a radical misplaced/mispunched date.

However, keep looking, there is a great RPM in the 1894 date.
