1992 quarter- multiple strike?

Does anyone have info on what caused this?

    Please help.. I found this quarter in my dads things after he passed. I can't find any info on this coin and would like to identify it.

    Hate to be a downer, but it looks like PMD to me, like someone used it for BB target practice or something.

    Looks like its been in a fire, the bubble is from heat and the blackish color kinda gives it away.

    Extreme heat will affect the clad layer on both the Obverse and Reverse.
    I think damaged. No evidence of multiple strikes.
