1964 d nickel with phantom 4?

    Yeah - just a stain that looks like a 4 - see the stain above the 6&4 is the same color?

    Perhaps, but you've also learned to control it, place logic over assumption, and have learned how coins are made... all of which make a big difference. That said, pareidolia is completely normal. ;)
