It costs $7.00 extra for attribution and you must write this on the 2X2 flip cardboard. You can download their forms online. It comes in 2 pages with all instructions on how to submit, fees, etc.

Aren't you supposed to submit using 2.5x2.5 mylar flips, not the cardboard ones? I recall all TPGs writing something along the lines of no responsibility for staple damage if you use cardboard?
Actually, PCGS will note major varieties that have a four-digit PCGS code without having to pay the attribution fee.
That's what I meant, sorry to not have been more specific. I'm so used to doing it I did not think to mention it that way.
Folks, IMO, if ANACS is now charging $7 per for WL 50c (when submitted with the 15 coin deal) they must really be hurting for business. Better take advantage of this deal while you can! Or...wait until they lower prices more. I was at a show in FL this weekend with two TPGS's set up. I know for a fact that ANACS took in about 1/5 of the amount of coins that the other service did based on the packages in the cases on their back table and the number of customers I saw at their table. Call me a sneaky observer. As for PCGS and NGC that are regarded as "the best." You get what you pay for as their slabs bring more money. Neither is hurting for business and they are expanding into foreign coins which has hardly been tapped. I believe the cost of the "parts" to make a slab are in the $4-$6 range for any TPGS. AFAIK, NGC and PCGS get their "parts" from china (that's probably why their slabs have been easily counterfeited) so they may even be in the $3-$4 range. Actual costs are probably a closely held secret. Return postage and handling probably generates some profit to.
I don't know exactly how ANACS does their insurance. But every time I sent in coins I was required to buy insurance. I never had a problem with lost or stolen coins so- did ANACS make a profit on my insurance charges?
