    The dark areas at the top of the head makes it difficult to tell what's happening.
    I've got two guesses:
    -- lamination
    -- inclusion

    Light the head from the top and a better analysis can be made.

    Procedures in Asia by a surgeon once involved putting a mans nose on his forehead for 5 months so the nose would retain its luster while operating whatevers behind a humans nose.

    It appears to be a die chip, to the point of being a interior die break. The condition of the coin leaves it with little value over face.
