Email them, send them the pictures, demand they correct their mistake and pay for postage both ways. You did your job, you paid for them to do a professional job and not screw it up.
I had a screw up with ICG. When I received the coins, they didn't seal the slabs together. However they did send me a label to send them back so it was no extra charge to me.
Looks like they need to hire someone to handle QC. Grading is subjective, but there's no excuse for shipping a coin inserted into the wrong holder. They should pay all expenses and put your order on fast track.
I do not see why you all would send it back to be "corrected"! I think this is a neat conversation piece. Mint errors are collectable, and people collect slabs. Together, you know what each coin technically graded, even if in the wrong holder.
The coins aren't in the wrong holders, the labels are. You'd think QC would pick up on something that obvious.
I work as a quality systems manager and therefore conduct a lot of root cause analyses for quality failures. One of the terms I like to use on occasion, which I think would fit here, is "rectal cranial inversion"...

Probably. At one time it was official (may only be semi-official now) and it was actually in their posing rules that "Thou shalt not criticize PCGS in any way, shape, or form." and posing something like this would get it pulled and possibly the poster banned.
I'm sure it happens often. I contacted them. They are fixing it and shipping it back to me for free and reimbursing me for my shipping and insurance expense to them.
Really? They screwed up my Variety Attribution by conveniently forgetting to put the attribution on the coin label! I returned it and even included the receipt for the postage. All I got was a voucher for a Standard Economy Submission! Now I gotta spend even more money to use that! Can you say $10 Handling Fee? (Sure you can! There! I knew you could!)
