Really worn, but not natural wear, more like sandpaper or abrasion of some sort. Another theory is that it was under a chair leg or machinery leg, the obverse has a weird "circle" on it, a chair leg or machinery leg would cause that, most likely just used as a washer to put under a loose table chair or machinery leg, the reverse would have been heavily pressed into the ground, maybe concrete due to all the wear. The rims are the highest points meaning they would have the strongest contact with the ground making them the most worn. That is just a theory not sure what happened but definitely post mint damage.
That circle looks like coin wrapping/sorting damage. The worn part looks like it might have gotten stuck in a dryer or washing machine for a long time, in one position, causing all that wear. Just another guess of course.
