Indian cud and off-center strike 1 rupee

Found this today in my rather small collection of indian coins. 1 rupee 2012. I have found a lot of other off-center indian coins. Most of them are no more pronounced than this one, but they are very common. Out of my 13 rupee coins, 5 were noticeable off-strikes.

Could be a misaligned die

 

The reverse is perfectly centered. What is a misaligned die? is that when only one side is off-center? how does this happen?
