How ? Easy, because the finalizer never even sees the coin in a slab. Nor do any of the other graders. The labels are printed, and the coins actually put in the slabs, by low level employees long after the coin has left the hands of the graders. But that is a good one. I like it almost as much as this one I used to own -

I don't know about PCGS but I do know that a grader does see the encapsulated coin at NGC before it goes out the door.
I have a 1914-D $2 1/2 Indian in a 1914 holder from pcgs. Not a super high grade coin, but I'm keeping it for the error
