I hope it is just dust on the outside of the holder, but I have seen holders that had "foreign" materiel in them though.
The graders don't encapsulate the coin though, especially at pcgs. Charmy (thepennylady) has written a article about her tour of pcgs, here is her picture about the encapsulation room:
