Looks like someone had to stamp a 5 on there for those that don't know Roman Numerals And it wouldn't be an error, it's called PMD.
If you're talking about the 5, that's damage. If it's the issue with what's going on from 1 to 4 o'clock, that might be detached lamination. A view of the obverse may help.
I think the issue in from 1 to 4 o'clock is where they clamped the coin to something when they counterstamped it.
