I wish I knew how also, but no...we paid someone to make it custom for him. If he likes it enough, we may get a matching Luigi for him as a Christmas gift.
ANA offers a course a course on Hobo Nickel Carving at Summer Seminar. http://www.money.org/Content/Media/974053f1a06049e49d5c4af15fdea820/2012SummerSeminarCatalog.pdf Summer Seminar is a little over a month away so you still have plenty of time to register.
How much did he charge you? I may want to get a few done from this person. This is mainly because back in the day, some old friends and my brothers (Me included) Would play mario. Each person had their own character and had fun. Now I want to give 'em coins to remember the fun times. Now I see that, I am very impressed with the design and want to have it done.
If anyone wants to pass on their email address to me, I'll forward them on to the artist who made mine. Just know in advance that his work is a bit pricey, especially for custom pieces, but IMHO it was well worth it. The nickel that he sent us was signed on the reverse and came with a small certificate of authenticity to validate his artwork, the date it was created and the name of the piece. The buffalo nickel, shipping & packaging was included in the price. He prices the pieces based on how much detail you're looking for and all of his nickels sell between $100 - $1000+. As you can see from the Mario, we chose to stay with a very simple design to keep the price down. Hope that helps!
