Part error?

    I know nothing about errors. But wondering if this is possible.

    Is there ever been an occurrence where a part of the die broke, and was smashed into another coin, or if some kind of nut or bolt fell out and was stamped onto the coin?

    Yes, it is called a strike-through. If the object is still there it is called a retained strike-through. If it's another coin it is called an indent.
