How do you tell if it's just plain paper or exceptional paper quality?

Most newer notes will have the exceptional paper quality. The difference between EPQ or PPQ (depending on the grading service) and non-EPQ is the feel/look of the paper. If it looks worn and ragged and/or could have been washed, pressed, or altered in some manner, it cannot get an EPQ rating. Here is a link to PMG that describes it in more detail: http://www.pmgnotes.com/news/viewarticle.aspx?IDArticle=414

To get an idea of the difference in hand, you will need to get experience with notes and experience handling them while actively thinking about paper quality of the note.
