Poor Washington. It looks like it's been run over, hammered, etc. If it is some kind of error, it's certainly unusual.

Is it a clad or silver?

 

Is it the same on the reverse? Or is the reverse ok.

If the reverse is ok, then I'd say you have half a magicians coin. Otherwise, looks like it was stuck inside machinery, or was just intentionally ground down.
