Two possibilities in my mind - 1 it's a fake slab, but need to actually see the slab to say. 2 - it was part of a bulk submission and the GOOD grade was assigned just like the BU grade is assigned in some bulk submissions.
Have to admit that I haven't seen that before but I doubt it is a mechanical error because they don't normally spell out Good. I think most likely Doug has the answer. It is VERY similar to the way they do bulk submissions that don't make the minimum requested grade.

There is a bunch the TPG's do with bulk submissions , which I never knew , like dipping coins . Live and learn.
