Mostly die deterioration issues. The lettering on the rim is from another coin being pressed into it, you can see it is backward, that didn't happen at the mint.
Is the last picture for the 1998?if so I'm pretty certain you found a WAM!!pictures of the FG on the 1998.

thought a mule is another coin over another coin, u can see on the nickel, it dont match up or looked doubled
