This is a fun one for the error folks to think through. This is a Canadian 1c error. First, we start with a split planchet (1.41 grams), you can see the striations on the reverse. Second, indented by another planchet, about 10% off-centre. All the product of a single strike. Equal and opposite pressures create the effects you see here. Oh, and lastly, enough of the obverse is shown to decipher it as a modified portrait, dating this error between 1979 and 1981.
