Yes, it is PMD! You can tell because the reverse has been totally obliterated from being pressed against a flat surface. If it were done at the Mint, you would expect to see the imprint of the reverse die.
I have only one thing to say to the coin destroyer, "A Dork is a Dork of course." PMD Melinda, sorry. Thanks for posting it.
If you think about the minting process, you can easily see that your coin would not have fit in the collar of the press so not a error from the mint, but PMD.
I'd say it was a vice job, done with a vise, or perhaps some one could devise a way to do it with a different device.
