Looks like de-lamination issue and PMD.

Thanks guys, I was thinking more than likely it was PMD, but the curled up medal around the hole just gives me the impression that something popped from the inside, and who could do that besides the mint?

Looks like someone tried to gouge a hole in it, but gave up before going through. Not a drill, nut some sort of sharp instrument.

Looks like something exploded to get out, like in the movie "Alien."

I would say it's damaged as in PMD and the wear from circulation shows around the hole as it's been smoothed.

PMD plain and simple made with a sharp tool or something but not a mint error

He's saying it's damage, not a mint error. Certainly somebody at the mint could have damaged it as such.
