Dang, nice one! Appears to be DDO-005, which shows strong tripling (not quadruple): http://varietyvista.com/12 Kennedy Halves/DDO Detail Pages/1964DDDO005.htm Check the reverse between N-I of United - it looks to have the prominent die gouge there as well.
The DDO you show in this post is from a 64D DDO-003. You must have made a mistake with the reverse you show, it is the same photo you used on your other post for the 64D DDO-005.
Oh, well that changes things a bit. Can you see the die gouge above the E of States? I don't see it in your picture, but doesn't mean it isn't there.
