As green18 says its a religious medal, and my guess is that it is intended for Arab Christians, since the names of the three holy cities are written in both latin and arabic letters.
From what Ive been reading, this is a Saint Francis of Assisi medal, the 3 places being the pilgrimage he took between 1211 - 1219 http://en.wikipedia.org/wiki/Francis_of_Assisi The Padre is - Father Quirico Porreca, OFM
