Julia Domna / Venus Rome mint die clash pair
A pair like this shows the error is damage to the die and not a restruck brockage which would be very unlikely to have exactly the same incuse placement.

Two coins from my own collection struck with the same obverse die but with different reverse types, even.

    Gordian AD 238-244 and Tranquillina.
    Roman provincial Æ tetrassarion, 25.1 mm, 11.78 g, 7 h.
    Thrace, Mesembria, AD 241-244.
Obv: ΑVΤ Κ Μ ΓΟΡΔΙΑΝΟC ΑVΓ CΕΒ-ΤΡΑΝΚVΛ|ΛΙΝΑ, confronted busts of Gordian III, laureate, wearing cuirass and paludamentum, and of Tranquillina, diademed and draped.
Rev: ΜΕ-CΑΜΒ-ΡΙ-ΑΝΩΝ, Demeter standing facing, head left, holding ears of corn in right hand, left resting on long torch.
Refs: Varbanov 4188; Corpus Nummorum Thracorum 9283.

Same obverse die, but the die had undergone some damage:

    Gordian AD 238-244 and Tranquillina.
    Roman provincial Æ tetrassarion, 28.6 mm, 11.36 g, 7 h.
    Thrace, Mesembria, AD 241-244.
Obv: ΑVΤ Κ Μ ΓΟΡΔΙΑΝΟC ΑVΓ CΕΒ-ΤΡΑΝΚVΛ|ΛΙΝΑ, confronted busts of Gordian III, laureate, wearing cuirass and paludamentum, and of Tranquillina, diademed and draped.
Rev: ΜΕCΑΜΒΡ-Ι-ΑΝΩΝ, Demeter standing left, wearing stephane, holding phiale and long torch.
Refs: Varbanov 4190, citing Karajatov 83.

Maybe also a double strike. The position of the K of ΤΡΑΝΚ below the bust is not the same and there is a large gap between the Γ and Ο of ΓΟΡΔΙΑΝΟC. Both could be explained by a second strike of the right side of the obverse.

As far as ancient coin abuse goes, I kinda like this AtG tet... not content with inflicting a couple of test cuts (including one that split Zeus's head in half), the thugs also bashed it so hard on its edge that it started to fold.
